As per the result derived from the analysis it is found that the NPV of the project is negative, hence the project should not be accepted. The company has an option that instead of setting the new production line, they can use the existing machine which is under utilized. As per the companyââ¬â¢s policy the machinery has to be depreciated along straight line method, so next year the written down value will become zero. Thus in the second year company has to purchase a new machine which will cost à £500,000. Thus the NPV and the IRR of the project will be as follows- The NPV of the project comes positive which indicates that the project will have sufficient cash inflow and it will be profitable in the long term. Hunting laws Essay Year after year in the hunting industry people have argued that crossbows should be legal for everyone during hunting season. Finally a few years ago the national hunting board of admnistratives passed a law stating, anyone can use a crossbow during the archery part of deer season. this new law take all the sport out of bow hunting, it makes illigally poaching deer tremendously easy, and also it teaches the newer generation of hunters the wrong way to hunt. As far back as hunting goes one of the hardest ways to kill an animal has been the bow. its an adrenaline rush pulling the bow back on a big whitetail. now their taking that thrill away by allowing the use of a crossbow. its taking all the sport out of bow hunting, with a regular bow a hard shot was 25 to 30 yards now thay have crossbows out that will shoot out to 85 yards. making it basicallly like a gun theres no pull back, its a trigger like a gun. as stated in NABC (North American Bowhunting Coalition) ( Crossbows are not bows. Crossbows have mostly rifle-like characteristics such as a gun stock for shoulder mounted shooting, trigger safety, optical sights, locked anc cocked at full draw by a mechanical device, carried loaded and ready to shoot, rigidly controlled internal ballistics, minimal movemnetn to shooting position and can be shot from a rest. ) Its not fair to the sport of hunting, used to it took a skilled archer to kill a deer, now a toddler could pull the trigger and kill one. The relaxed restriction on rossbow hunting makes poaching deer illigally much easier. Poacher have to fire off a gun, which was incredibly loud, which would scare deer off and let any game warden within 10 miles aware that something illigal was taking place. now we have givin poachers an easy route, a virtually silent killing machine. A poacher can kill a deer in a natioal park and no one would ever know they was there. However, some might say people could still poach with crossbow before they legalized them, and this is true, but now everyonne having crossbows in there truck on there way back from hunting puts a whole lot more pressure on the fact that they can take a long quite shot on a big deer on the side of the road. Its always in the back of every hunters mind when they see a deer on the side of the road, but now were giving every hunter the ultimate tool of being able to illigally shoot a deer with ease. Lastly but not least its teaching our newer genereation the improper way of hunting. Now were implying in our kids heads that its okay to shoot a deer at 80 yards with a crossbow. thats not right, they should hae to shot a regular bow and earn the deer instead of just pulling the trigger with these new crossbows. Its extremely inmportant to teach our kids good hunting skills like when to pul the bow back, how you have to make sure the deer isnt looking at you, but with corssbows it doesnt matter they just pull the trigger like a gun. I think we are abusing the right to use them. I think people with a physical disability should be the only ones legal to hunt with crossbows. not giving one to a teenager who just wants to go kill 20 deer just to say he did. Make kids and for that matter everyone go out and learn the right way with a real bow and arrow. I think we are ruinging the sport of hunting by allowing anyone use a crossbow to hunt deer. Its taking all the sport out of hunting, making illigally poaching deer like a walk in the park, and also teaching the newer genereations of hunters the wrong way to hunt.. There a lot of facts to support my claims, its a no brainer that the national hunting assosoation made the wrong decicsion by passing the law that crossbows are legal for anyone.

Analysis of Orthopeadic Theatre Time Utilization ANALYSIS OF ORTHOPEADIC THEATRE TIME UTILIZATION AT KENYATTA NATIONAL HOSPITAL Trauma is the leading causes of admission in Kenyatta National Hospital and often these patients require surgery. The trauma burden unpredictability usually mismatches demand and supply of the hospital thus posing challenge for systems improvement and streamlining. Utilization is defined as the ratio of time that an asset is used, against its capacity. For uniformity among the OR team, it is extremely important to developing a precise definition of turnaround time. For the purpose of this paper, turnaround time is defined as the time between incision close of patient n and incision open of patient n+1. This definition captures the surgeonââ¬â¢s, anaesthetist and nurse perspective of turnaround time and allows us to see the delay between the most expensive value-added times in the theatre. Previous study from KNH reported high rate of cancellation of elective list on the day of surgeries (20.6%)1. One of the causes attributed to these delays was inefficient utilization of OR time, mainly due to delay in starting time and long turnaround time within the OR. It was noted that 70.9% of non clinical cancellation was due to list overrun/lack of time. It was also the leading cause of cancellation in KNH at (43.8%). Three specialty were identified to have recorded the highest cancellation rate were Cardiothoracic surgeries 38%, Neurosurgeries 37.8% and Orthopedic surgeries 32.6%1. Cancellation generally increases waiting list and the risk of further cancellation, which may cause patient dissatisfaction and compromise patientââ¬â¢s safety2. Poor scheduling of operations can result in cancellation of operation which is costly to both the patient and the hospital3. With the escalating cost of healthcare, quality of care fails to meet expectation of our patients and therefore, the health care organization should look on strategies to improve quality while reducing cost of running hospitals. The efficiency of operating room can be measured in variety of ways4. Efficiency is defined in term of ability to translate available time into earning5ââ¬â8. Computer simulation and mathematical models, both of which essentially idealizations that the practical capacity for OR to be between 80-90%9. Justification Theatre complex is a high cost department with the hospital and therefore considerable resources are wasted if operating room if not used effectively. Improving the performance of operating room is key to achieving shorter waiting time for surgery, reducing cancellation for operations and more so achieving the implementation of booking of elective surgery in our hospital. Increased utilization of operating room improves patients flow and reduces the waiting list time. Improving theatre utilization would lead to a reduction in cost recovery from each patients10. 4 in their study in South African concluded that private operation theatre utilization rates were higher than public operation theatres due to commercial nature of private healthcare and absent consequent cost drivers in public health sector10. Utilization rate in South Africa was found to be 48% which was significantly lower than the globally bench mark of between 70-80%4. 11 concluded that the delay in starting list, under s chedule, interruption due to emergency surgeries, administrative reasons, induction of anesthesia and recovery police are the main factor that account for inefficient use of operating room facilities in India hospitals Operating room utilization rate is usually an indicator used in measuring efficiency in use of hospital resources. It is argued that high utilization is cost effective and improves quality of care10,12ââ¬â15. Brief Description of KNH Main Theatre KNH Main theatre is located in the first floor of the tower block adjacent to Critical Care Unit (CCU), Renal and Burns Unit. It has twelve operating rooms out of which two are allocated to Prime Care Center; one is dedicated for emergency surgeries 24 hours a day. The remaining nine are allocated to different specialty in a week. The specialties are General surgery, Urology surgery, Maxillofacial surgery, ENT surgery, Pediatrics surgery, Plastic surgery, Ophthalmology surgery, Orthopedic surgery, Cardiothoracic surgery, Neurosurgical and Obstetrics and Gynecology surgery. Within the OR the following equipments are found, Operating table in the center of the room, Operating lights directly over the table to provide bright light during surgery, Anaesthesia machine is at the head of the operating table equipped with Electronic monitor machine which records the heart rate and respiratory rate by adhesive patches called chest leads placed on patientââ¬â¢s chest, it has also the Pulse oximeter which is attached to the patientââ¬â¢s finger with an elastic band aid. Pulse oximeter measures the amount of oxygen contained in the blood, an automated blood pressure measuring machine that automatically inflates the blood pressure cuff on patientââ¬â¢s arm continuously recording blood pressure, there is the diathermy machine which uses high frequency electrical signals to cauterize blood vessels and cut through tissue with a minimal amount of bleeding. Other specialized machines may be brought in depending on the nature of surgery. In a week there are 45 theatre space slot allocated to different specialty, out of which orthopedic surgeries have 12 slots and the remaining 33 slot are allocated among the remaining 10 specialty. The orthopedic department is located on the sixth floor of the tower block and has four wards namely 6A, 6B, 6C and 6D. Ward 6B serves as a joint admission for all pediatric cases from 6A, 6C and 6D. Each ward has four days in a week to do the elective surgeries. Theatre procedure Each ward is expected to submit its operation list to main theatre reception before 15:00 hours on the day before its asà signed operation day. In the evening before the surgery, anaesthetists visit the wards to do a pre-anesthetic assessment on patients who had been scheduled for surgery. They are expected to re-assure these patients, assess their fitness for surgery, and also confirm the necessary investigation are done which they document in the pre-anesthetic check list. On the day of surgery porters are sent to the ward to bring the patients who are then received by a designated theatre nurse at the receiving area, the Receiving Area nurse check to confirm the identity of the patient, see that consent for the surgery had been given and all necessary investigation have been done. Thereafter, the patient is wheeled to waiting bay where he/she is kept on-hold until the theatre room is ready. All other procedures like intravenous access and induction of anaesthesia are done in theatre. After surgery, the patient is taken to Post Anesthesia Care Unit (PACU) while the operà ating room is cleaned in preparation for the next patient. Economic considerations also suggest that it is desirable to keep operatà ing rooms fully used. Thus, it is imperative that areas of time wastage in the theatre time flow be recognized and their causes identified. This will assist theatre managers take necessary steps to correct the problem14. Objective To identify areas of, and causes of operating room time delay, and suggest solution based on the identified deficiencies. Operating theatre represents an area of considerable expenditure in a hospital budget. Consequently, hospital administrators are so concerned with maximizing utilization of OR. This can only be made possible by knowing how much time is spent on which activity and there by identify the factors resulting in under utilization of OR16. Staffing KNH theatre complex is being managed by Assistant Chief Nurse (ACN) with the help of Administrative Officer. The two are answerable to the Head of Department (HOD) Theatre, TSSU and CSSD. The staff within each OR includes Surgeons, Anesthetist, 1- circulating nurse , 1- scrub nurse and 1- theatre sterile assistant (TSA). The general theatre activities are overseen by a Theatre Users Committee (TUC), with HOD as the chairperson. Other members are from Nursing, Surgery, Anaesthesia, Administration and Technical departments. Data Collection I will survey the timing of events in the Orthopedic OR using their elective operating lists available from Monday to Friday at theatre front office desk for a period of one month. I will use a designed proforma based on established operà ating theatre process steps to record the following; Patient sent-for (PS): Time when porter leave R/A to the ward for the patient Patient available (PA): Time the patient arrives at theatre R/A Patient in room (PIR): time when patient enters OR Anesthesia/Induction start time (AIT): time induction of anesthesia starts Surgery start time (ST): time cleaning of surgical area Surgery finish (SF): time dressing is put on the incision site Patient out OR (POR): Time at which patient leaves the OR From the above times, I will derive the following: Ward to theatre transfer interval: the interval between PS and PA (b-a). This interval is prolonged when it is greater than 20 minutes. R/A waiting time: It is the interval beà tween patient available (PA) and when patient is transferred to OR (c-b). It is prolonged if it is greater than 10 minutes. Pre-anesthetic waiting time: the difference between patientââ¬â¢s in room (PIR) and anesthesia/induction start time (AIT) (d-c). It is prolonged when it is greater than 20 minutes. Anaesthesia admission time (AAT): the interval between AIT and ST (e-d) and it is prolonged when it is greater than 20 minutes. Surgery duration: the interval between ST and SF (f-e). Anesthesia reversal time (ART): the interval between SF and POR (g-f) Turnaround time (TAT): the interval between incision close of patient n and incision open of patient n+1. There will be a one week pilot study to set the cut-off point for the intervals between normal and delayed time. This will be based on the mean time observed in the pilot study and for convenience it will be rounded-up to the nearest number divisible by five. In case of delay outside theatre the officer involved will be interviewed to determine the cause of the delay. However, delays that occures in the theatre will be observed directly by the research assistant filling the proforma. The data was entered into SPSS 11.5 which was used to calculate the time intervals and for statistical analysis Efficiency means the management of theatre time, costs resources and staff to undertake as many procedures as possible within given levels of resources, or doing the same number of procedures using a lesser amount of resources .

Wedding Speech ââ¬â Best Woman or Maid of Honor Good Evening! For those of you who donââ¬â¢t know me, I am Sarah, Frankieââ¬â¢s slightly older sister. I want to start by congratulating Frankie and Erik, and thanking all of you for coming here today. I of course have known Frankie, a very, very long time. Frankie and I have always been pretty much inseparable as long as I can remember and we have always managed to keep each other laughing. I vividly remember Frankie and I staying up until well past our bedtime, throwing stuffed animals at each other, talking about boys and laughing so hard at everything and anything. To this day Frankie is the only person I know that can make me laugh so hard it hurts. Now, I of course have lots and lots of fun embarrassing stories about herâ⬠¦but because it is her special day, I will leave her alone. But there is always Erikâ⬠¦Erik, Erik, Erik. The big bonus of Frankie and Erik dating as long as they have is that we have all gotten to know Erik really well, really, really wellâ⬠¦he has become the big brother I never hadâ⬠¦ or wanted. But honestly he has really become a part of our family. He hunts, drinks beer, and wears flannel so it wasnââ¬â¢t too much of a stretch. Now, I have been trying for weeks to come up with a single embarrassing story, something about meeting Erik for the first time, or the first time he met our family, and frankly I came up with nothing. Erik has always been polite, kind, and funny to everyoneâ⬠¦EVERYONEâ⬠¦frankly man itââ¬â¢s a little spooky.

They are perhaps on of the most interesting and colorful ecosystems to be found in the marine environment. They are very unique in many ways. Coral reefs play many important poles in the marine world. They must have certain conditions to be formed, and to survive. Lots of different sea life rely on reefs for habitat and sources of food, including some endangered species. Coral reefs are perhaps one of the greatest yet least known of wonders of our world. The formation of a coral reef is a long and slow process, taking about a year for a reef to grow about half an inch. Many factors affect the growth of the coral. The general pattern of coral reefs is a continuous cycle of growth and destruction, resulting from the growth of the coral and destructive activities of animals and storms. This pattern serves to keep the coral reef alive and at a fairly constant level. Coral reefs must have certain conditions to survive. They must be in reasonably shallow water. Low levels of sedimentation in the water are very important too, because too many sediments in the water can block the sunlight needed for the coral reefs. Because of the sunlight required for reefs to survive, they can only grow to depths of 45 meters deep. They only form in tropical seas and areas that stay above 20 degrees Celsius. The amount of oxygen is also important. Oxygen is produced in the daytime by plant photosynthesis, but the level drops dramatically at night. Ocean wa ves and water movement play a part in the amount of oxygen in the water as well, this allows diffusion of oxygen into the surface levels. There are three different kinds of coral reefs. There are fringing, barrier, and atoll. Fringing reefs are attached to margins of an island or continent, rough, table like surface, as much as 1 kilometer wide. On their seaward side they slope steeply to the ocean floor. Fringing reefs grow in shallow water near the shore and prefer dry climates with limited river runoff. There are also barrier reefs, which are similar to fringing but separated from mainland with a lagoon. The number of barrier reefs is greatly increasing due to global warming, because of the rise of the waterââ¬â¢s level.

Vermontââ¬â¢s famous ice cream markers, Ben & Jerryââ¬â¢s, started the City Churned campaign summer of 2013; where five major cities voted in traditional and non-traditional ways to create a unique flavor that capture elements of the city. The five major cities were Seattle, Portland, San Francisco, Washington DC and New York City. Ben & Jerryââ¬â¢s also teamed up with local favorites to make the flavor even more represent the city it is for. At the end of the summer, Ben and Jerryââ¬â¢s served the final product the city chose at community events. Ben & Jerryââ¬â¢s objective from start this campaign was of course to increase sales, but also to create a deeper connection was their consumers, Mike Hayes, Ben & Jerryââ¬â¢s assistant digital marketing manager, told Adweek. Another main objective of the City Churned campaign was to remind people of the social mission the company stands for and the importance of local roots. Their social mission is about keeping ingredients real (no GMOââ¬â¢s) and doing what is best for the planet at large. The company wanted to also focus on the importance of local roots by using products from companies local to that city. There were main three strategies that Ben & Jerryââ¬â¢s used in order to meet their objectives. The first strategy was having an interactive campaign. They made it so people were voting by doing everyday life activities, such as what turn gate you use when heading to train. The second strategy was taking a modern twist on crowdsourcing. They used the traditional method of having a website where people would directly vote right there, but also used new different ways to gather even more input. Ben & Jerryââ¬â¢s teamed up with transportation services popular to the city to count votes as well. In New York City, the cabs headed uptown counted as a vote towards peppermint, whereas a cab headed downtown was a vote Fairtrade vanilla. In Portland, they watched the men entering Cartopia and whether he had a mustache or aà beard was vote for different flavors. The last strategy they used was making local elements the main part of the campaign. This made locals to the city or people who knew the place interested in what was being picked. That created a lot of free press for Ben & Jerryââ¬â¢s. Local newspaper throughout the summer kept track of the voting and what was happening. Some of the techniques that Ben & Jerryââ¬â¢s used to accomplish the goal was repetition. Their name was all around the cities, especially at local hot spots. They also promoted the product being special to each city by capturing the local elements and flavors, and letting the people decide what flavors and elements really show the city. Another technique used was creating excitement, which created participation. The whole campaign was based around the people getting what they wanted. People took pictures are local monuments and used the hashtag for their city to vote and get into the campaign.
